About “Jockey Club End-of-Life Community Care Project: Unison”

Hong Kong is home to over 530,000 persons with disabilities (“PWDs”), approximately 70% of whom are aged 50 or above. Studies show that PWDs are more susceptible to secondary conditions, medical complications and premature ageing. To support PWDs with terminal or life-threatening illnesses who face various such challenges, The Hong Kong Jockey Club Charities Trust initiated and funded the “Jockey Club End-of-Life Community Care Project: Unison” (JCECC Unison) with an approved donation of about HK$290 million in 2024. This project aims to deliver individualised palliative and end-of-life care for PWDs and their caregivers.

“JCECC Unison” is a collaborative effort across multiple disciplines aimed at enhancing the capacity of professionals delivering palliative and end-of-life care, as well as raising public awareness through various training sessions, workshops and community activities. The service model is tailored to address the physical, psychological and practical needs of PWDs and their caregivers, both in the community and in residential homes.

Project Introduction

This project seeks to implement a collaborative approach to support families in need by partnering with hospitals, family members, patient support organizations, professionals, and community volunteers. It offers comprehensive end-of-life care to individuals with physical disabilities in terminal stages of illness and promoting their autonomy in end-of-life decisions. The initiative aims to improve patients’ quality of life, reduce caregiver stress, and nurture harmonious family relationships. Ultimately, it ensures that patients and their families live comfortably and with dignity during the end-of-life journey.

Project Objectives:

  • Provide home-based end-of-life support services for individuals with physical disabilities and their caregivers through a case management approach.
  • Strengthen family relationships and facilitate positive experiences for the families.
  • Organize and develop a volunteer team to support target service users and their caregivers.
  • Raise public awareness of end-of-life care and promote a positive attitude toward death.

Target Service Users:

End-of-life patients aged 18 or older with physical disabilities, along with their family members, with priority given to those residing in Kowloon.

Service Components:

  • Physical Care: Symptom management, caregiving skills training, rehabilitation exercises and training.
  • Psychosocial-Spiritual Care: Emotional counseling, life review, family relationship facilitation, wish fulfillment activities, care plan discussion and bereavement counseling.
  • Practical Support: Rehabilitation equipment lending service, home modification assessment, navigation of community resources and service referrals.
